Turn in your Health & Safety Paperwork

Everything is due July 15 Paperwork received after July 15 is charged a $15 late fee, and after July 31 a $25 late fee. Children cannot start school until their paperwork is complete. Step 2 is what takes time — book those appointments first.

Step 1 – Get the forms

Each form below links straight to its file, or you can open the whole folder. Start with the Forms Checklist — it tracks what you have finished and marks which forms are submitted digitally.

Step 2 – Book your doctor appointments

Several items need your child’s doctor, and appointments book out. Do this before anything else.

The Health Inventory part 2, Lead Testing Certificate and Immunization Form are for new students. The Emergency Form also needs a doctor signature at the bottom if your child has allergies, takes medication, or needs special care in an emergency — as do the Allergy, Asthma, Seizure and Medication forms if they apply to your child.

Some doctors no longer produce paper health records. Electronic copies are fine in that case.

Step 3 – Fill them in

New students

All 12 forms below, though several apply only if your child needs them.

Returning students

Everything except 5, 6 and 7 — those stay on file from last year.

  1. Forms Checklist 2026-2027 Start here. Tracks everything else and marks which forms are submitted digitally.
  2. Emergency Form 2026-2027 Doctor signs the bottom section if your child has allergies, takes medication, or needs special care in an emergency.
  3. All About My Child 2026-2027 fill in online Helps your child's teacher get to know them before the year begins. Nothing to download.
  4. Photo/Video Release Form fill in online Yes or no for each of five places a photo might appear. Nothing to download.
  5. Health Inventory Two parts - part 1 you complete, part 2 your doctor completes. New students only.
  6. MDH Blood Lead Testing Certificate From your doctor's office. New students only.
  7. Immunization Form Maryland immunization record from your doctor's office. New students only.
  8. Allergy & Anaphylaxis Form Doctor signature required. Only if your child has a food or other allergy.
  9. Asthma Action Plan Doctor signature required. Only if your child has asthma.
  10. Seizure Medication Form Doctor signature required. Only if your child has a seizure disorder.
  11. Medication Administration Prescriber signature required. Only if your child takes any medication at school.
  12. Topical Product Application Only if we need to apply sunscreen, lotion or similar.

If a section does not apply to your child, write n/a rather than leaving it blank. Save a copy for yourself on your own computer or personal Google Drive — not the school Drive, where every family can see it.

Step 4 – Send it all in one email

One email with everything attached, not a form at a time. Include a recent photo of your child — a new one each year. The Health Inventory, Lead Testing Certificate and Immunization Form must come in as paper copies; everything else can be sent digitally. A clear phone photo is fine as long as the whole page and any signature are readable.

Step 5 – Get your confirmation

The health and safety coordinator reviews what you sent and replies once your child’s file is complete. If anything is missing, they will tell you exactly what. You are not finished until you have that reply — so send early enough to fix gaps before July 15.

The coordinator role changes hands each year, which is why paperwork goes to the role address rather than to a person.
